Case Worker Client Enrollment Center

Case Workers, Court Appointed Guardians, Foster Parents can enroll a Minor or Adult needing counseling services here. 

To get started with counseling services, first fill out the secure Referral form. If the client receiving counseling is 17 or under in age fill out the Minor Referral form, for anyone that is 18 or older use the Adult Referral form. 

Once the form is completed and submitted, you will receive a copy of your completed form for your records. 

Once verified for services, you/client will be contacted to get scheduled with a therapist.
